RemedyLink Small Intestine Protocol

The goal is to reduce microbial load. This enables natural microbiome health, and fortifies immune defenses.

The Order of These Steps Matters

These three steps are not interchangeable. Bile is the germination trigger for the spores, so Step 2 does very little until Step 1 has landed. Bicarbonate is what allows pancreatic enzymes to work at all, and those enzymes clear the undigested substrate a small-bowel population feeds on.

Each step carries its own monitor, so the patient can tell whether it has taken hold before adding the next.

Step 1: Reinstate Bile

Bile is the small intestine’s own antimicrobial.

Bile acids disrupt the lipid membranes of bacteria and fungi, drive peristalsis, and trigger antimicrobial peptide production at the ileal-colonic boundary. Poor bile flow removes an entire defensive layer — and it is the most common reason a spore protocol appears to do nothing.

Monitor
Palpate GB41. Use firm pressure on the Gallbladder 41 acupuncture point. Tenderness indicates gallbladder stress. Track it across the course and expect it to soften as bile flow returns.

Gallbladder removed, or chronic biliary stasis?
There may not be enough bile to germinate the spores in Step 2. Take bile salts alongside them to supply the trigger. This is a workaround, not a fix. The full biliary workup can be found here: RemedyLink Gallbladder Protocol.

Step 2: Restore Spore Population

The transient Bacillus population the modern diet no longer supplies.

The healthy small intestine hosts transient, spore-forming Bacillus arriving continuously on food grown in living soil. Modern farming, sanitation, and kitchen practice have removed nearly all of that exposure. The spores germinate on contact with bile, produce antimicrobial peptides, disrupt biofilm, regulate local immunity, and contribute digestive enzyme activity.

Monitor
Stool buoyancy against fecal output. Read the two together, never buoyancy alone. Stool moving from floating to sinking while output holds or increases is the result you want. Stool sinking while output falls is the opposite reading. See Caution below.

Start Low and Titrate Up, Once Stool Sinks, That is Sufficient.
Going further is unnecessary and can be counterproductive. Excessive spore pressure suppresses the colonic fermenters the rest of the protocol is built to feed, and that damage takes weeks to months to undo.

Caution that sinking means the same thing in both directions - less gas trapped in the stool. An over-suppressed gut stops floating too, because there is less fermentation left to produce the gas. Output volume is what separates them, which is why the two readings are taken together and why this rule only works titrating up from a low starting dose.

Step 3: Activate Pancreatic Enzymes

Enzymes need an alkaline duodenum.

Pancreatic proteases, lipases and amylases digest for the host and, in doing so, remove the substrate a small-bowel population needs to establish. But they only work in an alkaline duodenum. Without the bicarbonate tide to neutralize gastric acid on arrival, supplementing enzymes accomplishes little - the pH is wrong for them to act.

Monitor Morning Saliva pH
First thing on rising, before food, toothpaste, or mouthwash. The salivary glands draw on the same bicarbonate reserve the pancreas does, so morning saliva reads the reserve available to alkalinize the duodenum. Full method and the pH ladder  can be found here: RemedyLink Saliva and Urine pH Protocol.

Supporting Measures

The Overnight Fast

The migrating motor complex sweeps the small intestine clean roughly every ninety minutes, but only during fasting. It needs three to four hours between meals, and its most thorough window is overnight. Twelve to fourteen hours costs nothing and is often the single highest-yield change on this page.

Gastric Acid

Hypochlorhydria from acid-suppressing medication, atrophic gastritis, or age lets a far larger inoculum through to the duodenum. One of the most common and most correctable contributors to overgrowth.

Ileocecal Valve

When incompetent, most often from surgical adhesions, dense colonic populations colonize the small intestine in retrograde. Topical castor oil or oral liposomal proteolytic enzymes can be considered for adhesions.

Nothing Works Against Re-exposure

A patient who eats continuously, or who is still on acid suppression, will not hold a result from any of the three steps above.

Where a bacterial target is confirmed: the polyvalent pyobacteriophage covers the gram-positive small-bowel players and the gram-negative ones in a single product, which makes it best-in-class against strep, staph, and the bile-resistant enterococcus. Phages are strain-specific, so a culture both confirms the target and guides coverage.
